STRIDE PHYSICAL THERAPY PRIVACY POLICY
- All patients will be asked to authorize Stride Physical therapy to send text messages verbally and in their new patient paperwork.
- We will not ask you for any personal information other than your name and phone number.
- Your personal information (name and phone number) will be used solely for the purpose of confirming your identity as a patient at Stride Physical Therapy.
- Stride Physical Therapy does not share ANY personal information with third parties or affiliates for marketing purposes.
- SMS consent is also not shared with third parties or affiliates for marketing purposes.
STRIDE PHYSICAL THERAPY TERMS OF SERVICE FOR SMS
SMS Consent Communication:
The information (name and phone number) obtained as part of the SMS consent process will not be shared with third parties for marketing purposes.
Types of SMS Communications:
If you have consented to receive text messages from Stride Physical Therapy, you may receive messages related to the following: appointment scheduling, appointment reminders, post-visit instructions, billing notifications and Google review requests.
Example: “Reminder you have an appointment at {appointment time} on {appointment date} with {clinic name}. To opt-out of receiving SMS messages at any time, simply reply “STOP” to any SMS message you receive.”
Message Frequency:
Message frequency may vary depending on the type of communication. For example, you may receive up to 2 SMS messages per week related to your appointments, billing, etc.
Please note that standard message and data rates may apply, depending on your carrier’s pricing plan. These fees may vary if the message is sent domestically or internationally.
Opt-in Method:
You may opt-in to receive SMS messages from Stride Physical Therapy in the following ways: verbally or by checking the appropriate box in your new patient paperwork. By opting into SMS, you are agreeing to receive SMS messages from Stride Physical Therapy, and your consent is not shared with third parties or affiliates for marketing purposes.
Opt-Out Method:
You can opt-out of receiving SMS messages at any time. To do so, simply reply “STOP” to any SMS message you receive. Alternatively, you can contact us directly to request removal from our messaging list.
