At Stride Physical Therapy we are happy to work with insurance to cover the cost of your care. If you do not see your insurance carrier listed, please call our office .

After you provide us with your insurance information, we will contact them to learn the specific benefits of your plan, what services have been used this year, and if there are any limitations on therapy services. If your carrier requires a co-pay, deductible or co-insurance payment we will notify you of that at the time of your first visit. We will also verify whether your carrier requires a referral. If required, we ask that you provide your referral at your first appointment to avoid any billing problems.

If you DO NOT have insurance or a third party is responsible for payment of your injury, please let us know when you schedule your first visit so we can discuss payment options prior to your evaluation.
